Best of all, this manual includes sample forms, documents, policies and a sample practice protocol adapted for inpatient settings. All are intended to facilitate the credentialing and privileging process, and may be reproduced and modified for the exclusive use of the purchaser.

Sample Protocol
This is a model for the document that is required by Texas law when a physician delegates prescriptive authority and other aspects of medical care to an advanced practice nurse (APN). A more descriptive term is “practice agreement." However, “protocol” is the term used for this document in Texas law. This document does not contain any treatment guidelines for specific diseases. It contains the delegation statements that have to be included and has many explanatory notes and citations to help APNs and physicians understand the law.
The sample protocol is in a Microsoft Word (.doc) file.
